Documents Needed for a Road Test

You must always check for these documents and double check them. All originals!

 

Schedule a Road Test Appointment: All Road Exams must be scheduled online with the Illinois Secretary of State. All Road Test appointments open up at 6:30 AM each Day. Everyone must schedule under “New DL or Instruction Permit” (For Teens who took the Cooperative Driving Test Program, they must schedule under “Change Instruction Permit (Teen) to a DL”. Keep in mind the students will have to type out their DL number and Date of Birth. Also, the two requirements needed to schedule online are to hold Instruction Permit for minimum of 272 Days AND be minimum age of 16). If anyone schedules under any other categories, your appointment will be cancelled. Keep in mind appointments go real fast.

Teenagers: Ages 16-17

  1. Valid Illinois State ID or Unexpired Passport or US Birth Certificate

  2. Social Security Card (Original and Signed)

  3. Proof Address (Parent can sign for this)

  4. Instruction Permit (Original and held for 9 Months Minimum, obtain at 15 with permit application)

  5. 50 Hour Log Sheet (10 Hours have to be logged at night)

  6. Certificate of Completion or Blue Slip: You can check status at: ilsos.gov/deccheck

  7. Vehicle Car Insurance

  8. Optional: Notarized Affidavit if parent is not present when taking the road exam.

  9. Optional: Cooperative Driving Test Waiver (Pink Form). Please note there may be a random spot check. THIS PROGRAM ENDED ON DECEMBER 31, 2022 FOR ALL DRIVING SCHOOLS!

  10. Optional: Cooperative Driving Test Program is effective again but it is updated to the driving record. There is a random spot check. Odds are 3/31 or 9.6% chance. Dates are picked 3 days out of the month as random.

 

Adults 18 Years Old+

1.) Student who is an American Citizen and has a State ID (Non-TVDL):

  1. Unexpired Illinois State ID

  2. Signed Social Security Card

  3. Instruction Permit

  4. Car Insurance is provided by the instructor

  5. Optional: Proof of Address if changing address.

  6. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. The student could verify their status at ilsos.gov/deccheck if students took Driver’s Education in High School or Commercial Driving School.

  7. Optional: If student does not have a permit, they must either bring an out of state unexpired driver’s license or a foreign driver’s license with their passport stamped no later than 90 days from the latest date of entry into the United States. This is required as proof they drove before. An Expired Illinois Instruction Permit or Any Out of State/Foreign Instruction Permits are not valid.

 

2.) Student who is an American Citizen and has no state ID (Non-TVDL):

  1. A Form with their signature (Credit Card, Passport)

  2. A Piece of current official mail dated no less than 90 days or lease or bank statement.

  3. Proof of Birth such as a Birth Certificate or Passport.

  4. Signed Social Security Card

  5. Instruction Permit

  6. Car Insurance is provided by the instructor

  7. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. The student could verify their status at ilsos.gov/deccheck if students took Driver’s Education in High School or Commercial Driving School.

  8. Optional: If student does not have a permit, they must either bring an out of state unexpired driver’s license or a foreign driver’s license with their passport stamped no later than 90 days from the latest date of entry into the United States. 3.) Student who is a Permanent Resident (Green Card Form I-551) with a State ID (Non-TVDL):

  1. Unexpired Illinois State ID

  2. Signed Social Security Card

  3. Instruction Permit

  4. Car Insurance is provided by the instructor

  5. Unexpired Permanent Resident Card (Green Card). If expired, they can bring their extension letter. Usually it’s for 12 months to 18 months.  

  6. Optional: Proof of Address if changing address.

  7. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. The student could verify their status at ilsos.gov/deccheck if students took Driver’s Education in High School or Commercial Driving School.

  8. Optional: If student does not have a permit, they must either bring an out of state unexpired driver’s license or a foreign driver’s license with their passport stamped no later than 90 days from the latest date of entry into the United States. This is required as proof they drove before. An Expired Illinois Instruction Permit or Any Out of State/Foreign Instruction Permits are not valid.

 

4.) Student who is a Permanent Resident and has no State ID (Non-TVDL, Non-Dependent):

  1. A Form with their signature (Credit Card, Passport)

  2. A piece of current official mail dated no less than 90 days or lease or bank statement.

  3. Proof of Birth such as a Passport (Just in case).

  4. Signed Social Security Card

  5. Unexpired Permanent Resident Card (Green Card). If expired, they can bring their extension letter. Usually it’s for 12 months to 18 months. 

  6. Instruction Permit

  7. Car Insurance is provided by the instructor

  8. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. 5.) Student who is on a Student Visa (F-1, J-1, M-1, I-20 Visa) with Social (Non-TVDL, Non-Dependent)

  1. A form with their signature (Credit Card, Passport)

  2. Proof of Birth such as a Passport.

  3. I-94 printed out with their recent date of entry into the country. NOTE: The date of last entry needs to match what is on the passport (Note: I-94 might be on the 797A)

  4. I-20 or Other Visa like a 797A form (Note: I-94 might be on the 797A)

  5. Signed Social Security

  6. A piece of current official mail dated no less than 90 days or lease or bank statement.

  7. Instruction Permit

  8. Car Insurance is provided by the instructor

  9. Optional: unexpired Illinois State ID if you got it for proof of address.

  10. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. The student could verify their status at ilsos.gov/deccheck if students took Driver’s Education in High School or Commercial Driving School.

  11. 6.) Student who is on a Student Visa (F-1, J-1, M-1, I-20 Visa) with no Social (TVDL, Non-Dependent)

  2. Proof of Birth such as a Passport.

  3. I-94 printed out with their recent date of entry into the country. NOTE: The date of last entry needs to match what is on the passport (Note: I-94 might be on the 797A)

  4. I-20 or Other Visa like a 797A form (Note: I-94 might be on the 797A)

  5. A piece of current official mail dated no less than 90 days or lease or bank statement.

  6. Instruction Permit

7.) Student who is on a Work Visa (H-1B), L1, L1A or O-Visa with Social (Non-TVDL, Non-Dependent)

  1. A form with their signature (Credit Card, Passport, Unexpired Illinois State ID)

  2. Proof of Birth such as a Passport.

  3. I-94 Printed out with their recent date of entry into the country. NOTE: The date of last entry needs to match what is on the passport (Note: I-94 Might be on the 797A)

  4. H-1B is usually on the 797A form. (This is known as a petition) (Note: I-94 might be on the 797A)

  5. Work Authorization card (EAD) if present. This is Form I-766 or I-688B

  6. Signed Social Security Card

  7. A piece of current official mail dated no less than 90 days or lease or bank statement.

  8. Instruction Permit

  9. Car Insurance is provided by the instructor

  10. 9.) Student who is on a Dependent Visa like H-2B, H4, J-2, F-2, etc Visa (TVDL, Dependent)

  1. A form with their signature (Credit Card, Passport)

  2. Proof of Birth such a Passport.

  3. Letter from Department of Homeland Security verifying they are now able to get their driver’s license. It would be a letter sent from the Illinois Secretary State. Please note that if the letter is over 30 days old, they require re-verification. This takes anywhere from 5-10 Business Days.

  4. I-94 Printed out with their recent date of entry into the country. NOTE: The date of last entry needs to match what is on the passport (Note: I-94 Might be on the 797A)

  5. These dependent visas are most likely on their 797A form. (This is known as a petition) (Note: I-94 might be on the 797A)

  6. A piece of current official mail dated no less than 90 days or lease or bank statement.

  7. Instruction Permit

10.) Student is on a Tourist Visa like B1-B2 Visa. Note: Social Security would make them Non-TVDL, No Social Security would make them TVDL.

  1. A form with their signature (Credit Card, Passport, Unexpired Illinois State ID)

  2. Proof of Birth such as a Passport.

  3. I-94 Printed out with their recent date of entry into the country. NOTE: The date of last entry needs to match what is on the passport.

  4. Immigration Documents. Passport with visa might suffice.

  5. Signed Social Security Card if present

  6. Work Authorization card (EAD) if present. This is Form I-766 or I-688B

  7. A piece of current official mail dated no less than 90 days or lease or bank statement.

11.) Student who is on DACA also known as a Dreamer (Non-TVDL)

  2. Proof of Birth such as a Passport

  3. Work Authorization Card. This is Form I-766 or I-688B

  4. Signed Social Security Card

  5. A piece of current official mail dated no less than 90 days or lease or bank statement.

  6. Instruction Permit

  7. Car Insurance is provided by the instructor

  8. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. 12.) Student who is undocumented (TVDL)

NOTE: Tests can only be done Monday through Friday. No Saturdays!

  1. A Form with their signature (Credit Card, Passport, unexpired matricula (Consulate Card))

  2. A Piece of current official mail dated no less than 90 days or lease or bank statement (Matricula can be used if current).

  3. A piece of Official Mail dated over 12 months like a lease or Bank Statement. This is for proof they have lived in Illinois for 12 months. Note: Matricula may only be used once.

  4. Proof of Birth such as a Passport or Matricula.

  5. Instruction Permit

  6. Car Insurance is provided by the instructor

  7. TVDL Residency Form Filled Out (May be filled out at DMV)

  8. Optional: Anyone between the ages of 18, 19, or 20 are required to either have a Six Hour Adult Certificate or a Certified High School Transcript with proof they obtained a passing grade in Drivers Education Classroom. The student could verify their status at ilsos.gov/deccheck if students took Driver’s Education in High School or Commercial Driving School.

  9. Optional: If student does not have a permit, they must either bring an out of state unexpired driver’s license or a foreign driver’s license with their passport stamped no later than 90 days from the latest date of entry into the United States.       REAL ID Documents

Starting May 7, 2025, Illinois will be fully REAL ID-Compliant. They will need to obtain a REAL ID Driver’s License or REAL ID identification card to board domestic flights, enter military bases, or secure federal facilities. Starting February 1, 2026, there will be an imposed $45 Penalty Fee for flying without a REAL ID.

Who gets this? American Citizens (Naturalized Forms N-560, N-561, N-550 or N-570); Permanent Residents (Forms I-551); Work Authorization Document (EAD Form I-766 or Form I-688B); and Foreigners that have a foreign passport with Valid Visa affixed with the I-94 Form.

Group 1: Proof of Identify, Date of Birth, and U.S. Citizenship or Lawful Status.

Documents can be: Birth Certificate, Passport, Work Authorization Card, Permanent Resident Card, Foreigners with valid Visa, I-94 and Social Security.

Group 2: Full Social Security Number (SSN)

Documents can be: Social Security Card, W-2, or Pay Stub with Social Security Number and Applicants Name.

Group 3: Residency (Need two proof of addresses. Must be printed out. Nothing Digital)

Documents can be: Utility Bill, Lease/Mortgage, Voters Registration Card, etc.

 

Note: Teens who are 15, 16, or 17 can obtain a Real ID by having Group 1 and Group 2 under Teen’s name, but Group 3 can be the parent’s two proof of address.
